Submit Button Not Working

Hi fellow members, can someone direct me on why my submit button is not working? My form was created in MS Office Publisher 2003, my field properties is set for the my_name@bluehostdomain.com email account.

Getting the following Windows Internet Error
HTTP 404 NOT Found
Most likely causes:
There might be a typing error in the address.
If you clicked on a link, it may be out of date.
What you can try:
Retype the address.

Go back to the previous page.

Go to and look for the information you want.

What do you mean not working? What do you want it to do? Where do you want it to go? Do you have a form handler?:)
