Phantom Guests

We keep finding one, sometimes two guests in sub-forums we have set with restricted access. Are these actual folks getting through the security perimter or are they "phantoms".

I read a while back that phantom quests are a possibility but how can one be sure?

Yes it is phpBB version 3.0.8...sorry I did not make that clear.
