Hi! I need help making a script, I'm not sure how it's called but I can try to explain it.

Here's a sample shot of it.
I want viewers to be able to submit links and have it appear right away on another page in my website. Kind of like a petition signing but with links. For example, in the photo the viewer can put in the title of the link (or how they'd refer it to as) and then put the link in the next box, choose a category for it, and then submit it. then in another page it'll automatically show the link.
Can anyone help me set this up or point me in the direction to it? I've been searching online for hours for help on this =(
