I think you are describing an addon domain as described in this article: http://helpdesk.bluehost.com/kb/index.php?x=&mod_id=2&id=372
To setup email the process is exactly the same as you setup an email account for your master domain, just make sure you select the correct domain name in the email setup screen when setting up email accounts for pointed/addon domains.

I have the same question about addon domains and email setup -- I was hoping to set up the email before transferring the nameserver for the addon domain. However, the process does not appear to allow for that option. It looks like I have to establish the new nameserver, thus transferring the addon domain, and then after it is connected, then I can setup emails for this addon domain.
Please advise.

I opened up a live chat session. The operator understood that my concern is not losing email during transfer of addon domains. Unforturnately, the current restriction on addon domains is that they need nameserver transfer in order to connect them to the main domain through the control panel.
The operator offered to "manually add" (meaning tech support adds them, instead of the subscriber using the control panel) these addon domains so that I could establish the email accounts. That way, when the nameserver switch occurs, there would (hopefully) not be any lost or bouncing emails.
A long-term solution would be to allow these added domains to be established even in advance of nameserver resolution. The solution is possible, and has been something I have used at other hosting providers in the past. The "manual add" solution is a workable patch for this concern about not losing emails.
