Need help application form

On our website, usmcmta.org, we have a button called "application" I have received comments that some users are seeing html code instead of the application form.

I reloaded it after renaming it and it continues to be a problem. It seems as though Fire Fox is ok when viewed there, but when using Safari, it reads as html code.

Does anyone have the answer to this problem?

It's showing as html code in Firefox too now... you need to lose the ".txt" from the filename. Don't have time to look at your code right now, but will check it over later if nobody else has sorted it. :)
