Domain registration at Bluehost is $10 per domain per year. Subdomains don't cost anything since their names aren't registered separately.

You don't have to purchase the additional domains from BlueHost, you can purchase them from anywhere that sells domains and point them to the BlueHost nameservers to use them with your hosting.

For top level domains that BlueHost don't sell you would have to buy them elsewhere.
